Registration of Birth

It is essential to register a child who was born outside Sri Lanka, at the Embassy of Sri Lanka prior to obtain a new passport and apply for citizenship certificate.

Registration of births should be made within 3 months of birth. Where an application for registration of a birth is made after one year from the date of birth, the reasons for the delay should be submitted to the Embassy along with the application. 

The birth should be registered within a period of one year from the date of birth of the child. Delayed Birth registration will be processed after receiving the approval  from the Registrar-General’s Department, Sri Lanka.

For the registration of birth, the following documents should be submitted:


(1)    Duly filled Application Form CR1, (if the application is made within 03 months of birth)

Duly filled Application Form B6 (if application is made after 03 months of birth)

(2)    Duly filled application for registration of birth outside Sri Lanka

The forms mentioned in (1) and (2) above should be completed in duplicate.

3)    Declaration of citizenship by father / mother (This form should be filled in triplicate.)

(4)    Birth certificate of the child (original) issued by the competent local authority in the country of birth of the child

(5)    Birth certificates (Original) and passports of the parents
 
(6)    Marriage certificate (Original) of the parents

Note:

Relevant sections of B4/B6 forms, declaration forms, citizenship form should be attested by Notary Public/Lawyer prior to submission

Fees payable for registration of birth are as follows:
Registration of birth - RMB 160.00 /USD 25.00
Issuance  of certified copy - RMB 85.00 /USD 13
Issuance  of citizenship certificate –RMB 300.00/ USD 47.00
Postal charges (if applicable) RMB 30
